Ohio Med Spa Drug License Suspended - Multiple Drug Safety Violations

Inspectors inspecting a medical spa
An Ohio medical spa with an impressive online reputation is now facing serious regulatory scrutiny after state inspectors uncovered unapproved injectable drugs, expired medications, unsanitary storage conditions, and repeated violations of state pharmacy laws. On September 15, 2026, the Ohio Board of Pharmacy suspended the “Terminal Distributor of Dangerous Drugs” license held by iBeauty MedSpa in Sylvania, Ohio, citing practices that posed a danger of immediate and serious harm to others. What makes this case particularly concerning is that several of the violations were not new. Inspectors had identified similar issues during a previous inspection in November 2025, raising questions about the facility’s pharmaceutical handling practices and supplier verification procedures. Following its September 10th  inspection, the board issued a 17-page notice detailing the findings and ordering the immediate suspension of the facility’s drug license.

What Did the State Board Found During Inspection?

  • Unverified Trading Partners: During the inspections it was found that the Med Spa was buying products from a non licensed wholesaler ‘Alpha BioMed’ which is not authorized to distribute drugs in Ohio.
  • Unapproved injectable drugs: Dozens of vials containing unapproved and illegal products, including medications purchased from a supplier the board identified as lacking the required licensing.
  • Expired medications: Drugs with expiration dates dating back to June 2023 remained in active inventory.
  • Improper drug handling: Pre-filled semaglutide syringes lacked appropriate labeling and preparation records.
  • Unsanitary storage: Inspectors reported dirty medication refrigerators, sticky drug containers, and missing temperature monitoring on the mini fridges.
  • Repeat violations: Four violations had previously been identified during a November 2025 inspection, including improper handling of drug vials and other violations.
The medical spa is challenging the suspension, with an administrative hearing scheduled for January 6, 2027.

What are the Most Common Mistakes by Med Spa?

  • Purchasing from unlicensed suppliers: Failing to verify whether drug wholesalers and suppliers are properly licensed.
  • Using unapproved medications: Purchasing or administering injectable drugs that lack required FDA approval including non approved compounded drugs.
  • Poor drug recordkeeping: Failing to maintain accurate purchasing records, medication documentation, and applicable drug tracing information.
  • Improper medication storage: Keeping expired medications, failing to monitor refrigeration temperatures, or storing drugs in unsecured areas.
  • Improper labeling and handling: Using unlabeled syringes, failing to track medication expiration dates, or improperly preparing injectable products.
  • Misunderstanding DSCSA requirements: Assuming medical spas are automatically exempt from federal drug traceability obligations without determining which requirements apply.
Compliance is not optional when regulations apply to your operations. Medical spas may be subject to inspections and audits by state pharmacy boards and other regulatory authorities, while applicable DSCSA requirements may involve maintaining drug traceability records for inspection. Staying ahead of compliance requirements helps protect your patients, your reputation, and your business from avoidable regulatory risks.

InfiniTrak Had Reached Out a Year Earlier

What makes this case especially significant to us at InfiniTrak is that our team had approached dozens of medical spas around the greater Toledo area including iBeauty Med Spa on January 24th, 2025, to educate them about the Drug Supply Chain Security Act (DSCSA) and the responsibilities that apply to a med spa operations under federal law. During our outreach, we discussed the importance of pharmaceutical traceability, responsible drug sourcing, supplier verification, and DSCSA compliance. The iBeauty Med Spa facility chose not to move forward with InfiniTrak, which is natural in business. However, what disappoints us is despite being informed about the potential compliance requirements, the facility did not implement any form of DSCSA compliance procedures. This reflects a misconception we frequently encounter in the medical spa industry: the assumption that pharmaceutical compliance is only relevant to traditional pharmacies. A medical spa is still accountable for the medications it purchases, handles, and administers. Being a medical spa does not place a business outside the requirements of the law.
